Just this week, a 25,000-person study presented at the European Society of Cardiology Congress in Munich suggested that people on the lowest-carb diets had the highest risk of dying from cancer, cardiovascular conditions, and all other causes. Another study, published this month in the Lancet, also found that people who followed diets that were low in carbs and high in animal proteins had a higher risk of early death compared to those who consumed carbs in moderation. What about fruits and vegetables? All fruits are rich in carbs, but you can have certain fruits (usually berries) in small portions. Other causes of diarrhea on the keto diet include consuming a diet low in fiber (fiber helps ward off diarrhea by bulking up stool) and eating processed low-carb foods like shakes and bars that may contain sugar alcohols. These sugar alcohols can ferment in the gut and cause gastrointestinal discomfort. Yawitz suggests limiting foods labeled “sugar free” if you’re prone to gas or diarrhea when you eat them. Eliminating these bad carbohydrates might work because you’re actually eliminated a huge source of heavy metal poisoning. Research done by Renee Dufault of the Food Ingredient and Health Research institute strongly suggest that foods laden with preservatives and mercury-laced processed food additives have a strong causal effect on health conditions ranging from ADHD to diabetes. If you are eating processed foods in the U.S., you’re getting a steady diet of heavy metal poisoning from the preservatives, pesticides, and concentrations of mercury in the U.S. food supply. If “food” can sit on a shelf for long periods of time without spoiling, there’s some kind of preservative in it that just might eventually kill you by triggering whatever dis-ease you are genetically predisposed to acquiring. It builds mitochondria. Mitochondria are the power plants of our cells, transforming incoming nutrients into ATP. The more mitochondria we have, the more energy we can utilize and extract from the food we eat—and the more performance we can wring out of our bodies. Ketosis places new demands on our mitochondria, who adapt to the new energy environment by increasing in number.
As was described in a recent Diet Doctor post on using ketones for Alzheimer’s Disease, it has long been known that the brain can use two fuels for its energy needs: 1) glucose or 2) ketones. Glucose is the product of the breakdown of carbohydrates that we eat or is made via our liver by a process called gluconeogenesis (literally “new glucose making.”) Ketones are the product of the breakdown of fat to fatty acids, either from fat in our diet or fat stored in our adipose tissue.
At some point, the pancreas is literally burned out and can’t produce enough insulin to keep even basic blood sugar under control, let alone after a high-carb meal. This state is end-stage of type 2 diabetes where insulin injections become necessary [7]. Type 2 diabetics do not produce less insulin than normal, but the insulin resistance increases the need for insulin. A diabetic pancreas is eventually unable to supply this increased demand.
He was wrong. Originally used for epilepsy, studies show that keto diets are far from dangerous. In fact, they benefit numerous conditions, including cancer, dementia, and overweight/obesity, and modern history only supports these claims. During the early 19th century, physicians had dramatic success implementing keto diets, yet they eventually found that prescribing drugs was much easier that than scrupulously monitoring what people ate. In order for this process of fat breakdown to ‘work’, the protein content must be kept low enough to prevent gluconeogenesis. So, just because you are eating a low carbohydrate diet, does not mean you are in ketosis. It is important to note here, that this nutritional ketosis is different from ketoacidosis, which is the setting of low blood pH level that occurs in people with diabetes and can be very dangerous.
While many of the problems that develop with prolonged ketosis may be addressed simply by minding intake of prebiotic fibers, not all are, such as selenium deficiency and stunted growth. (Note that the ideal intake of prebiotic fibers, the level we aim for in Wheat Belly and Undoctored programs, is 20 or more grams per day.) Some have argued that higher beta-hydroxybutyrate ketone levels that develop with a ketogenic diet is all you need to do to maintain healthy bowel flora, but this is a huge extrapolation that does not make sense in light of the newest insights into the microbiome and its metabolites. It ignores the role of hundreds of other microbial metabolites that are required and/or produced that are changed with prolonged deprivation of prebiotic fibers. Also, some have blamed the adverse long-term effects in kids on the seizure medications they take, but the side-effects of, say, drugs such as tegretol, valproic acid, or topiramate do not include the above phenomena.

In order for your body to turn fat into energy, it has to be in a state of nutritional ketosis. You can induce your body into this condition through a ketogenic diet — a dietary approach that focuses on three key points: high consumption of healthy fats, moderate intake of high-quality protein and minimal amounts of carbohydrates.

Walking, stretching, or doing gentle yoga or other mind-body exercise should be fine and may even help you feel better. But when your body is already under stress from trying to adapt to a new fuel system, don’t place an additional burden on it by attempting any type of strenuous workout. Take it easy for the first few weeks and then slowly increase your exercise intensity.
The keto diet may also be beneficial for adults with epilepsy, though the Epilepsy Foundation notes that it’s less frequently recommended for this group because it is so restrictive and difficult to stick with. (1) One study, published in May 2016 in the journal Epilepsy & Behavior, found the diet reduced the frequency of seizures for many study participants, 7 percent of whom were seizure-free at the four-year mark. (2) And though it wasn’t the goal for this study, nearly 20 percent of the participants lost weight while following the diet.

Fatty acid production in fat tissue is stimulated by epinephrine and glucagon, and inhibited by insulin. Insulin is one of the hormones the pancreas secretes in the presence of carbohydrates. Insulin's purpose is to keep blood glucose levels in check by acting like a driver, pushing the glucose into cells. If insulin were not to be secreted, blood glucose levels would get out of control.
I, too, am finding the keto diet to be beneficial. My weight is moving down. My recent A1c was 5.7. I am consistently below 90 each morning when I check my blood. The most important thing that diabetics have to learn is that there is no reason to avoid fat, not even saturated fat. Here’s the tricky part: There’s no definite answer for how much protein you’d have to eat before you run into trouble. “It really depends on how much protein a person is consuming versus how much they need, as well as the health of their kidneys at baseline,” Hultin says. That’s why it can be helpful to speak with a nutritionist or doctor who can help you tailor your diet before going keto.

I think you meet to cite some of the other research out there I’ve read in other books and the work the Virta Clinic has done. In my opinion you’ve cherry picked data to suit your preconceived beliefs.

Research is continuing. This was just a pilot study,1 Dr. Saslow says, so we could test the effects in a small group in order to see if working with patients online offered an effective way to have people follow a weight loss program. I would love to join a study! Could you recommend where to go to find one? I have done 30 or less total carbs a day for almost 2 years and feel great. I have NEVER EVEN ONCE gone over 40 total so I do follow it and I do not cheat on high carb foods, although I do occasionally over-eat on low carb foods resulting in a bit over 30 maybe 1 -2 times a month. I do feel that time will provide more support and think that the medical community should educate on this as another alternative. It isn’t for everyone because it is a bit more problematic if a person does fall of the straight and narrow too often but it can be done!

There is a reason why we store hundreds of thousands of calories in the form of fat in our body and only about 2000 calories in the form of glucose (with only a small amount of this useable by the brain). If you’re a part of our community on Facebook, you may remember seeing a study I recently posted regarding the long-term impacts of the ketogenic diet in a study of obese patients. The study, available thanks to the US National Library of Medicine and the NIH, had some very interesting findings on the impact of diet on brain and body health, and my post on the subject saw a lot of traction. Because of that, and because the findings have such importance, I wanted to share the research with you here. The study shows that the ketogenic diet has efficacy as a treatment for Alzheimer’s, epilepsy, depression, migraines, and many other illnesses that plague huge portions of our population. Additionally, people with gallbladder disease, pancreatic insufficiency, a history of kidney stones, or a history of anorexia should talk with their doctor regarding the safety of a ketogenic diet program. Last, women who are pregnant or breastfeeding require higher protein intake and should adjust the ketogenic diet plan to meet this macronutrient need.
“Some patients may need to supplement with sodium, as long as they do not have blood pressure issues. Some may even need prescription potassium supplementation,” Rahnama said, adding that she begins all keto diet patients on a magnesium supplement, as it’s an electrolyte that can be taken with low risk of overdose. She also said keto dieters may have to up their carb intake if they have continued issues with hydration.
“Keto is not a great long-term diet, as it is not a balanced diet,” says Nancy Rahnama, MD, MS, an internal medicine and bariatric specialist in Los Angeles. “A diet that is devoid of fruit and vegetables will result in long-term micronutrient deficiencies that can have other consequences. The keto diet can be used for short-term fat loss, as long as it is under medical supervision.”

One study from 2005 followed 22 people with Type 1 Diabetes for 12 months. The difference here however is they consumed 70-90 grams of carbohydrates per day versus the restrictive less than 20 grams per day on the Ketogenic Diet. Remember my motto? Moderation is the key! The results were positive; less hypoglycemia, insulin requirements were reduced and their A1c dropped from 7.5% to 6.4%.
