3. Ketone bodies are NOT a better source of energy than glucose. Your body uses SO much more energy just to MAKE ketone bodies. Basically, your cells take in fats and turn these into Acetyl CoA in the mitochondria. In a NORMAL cell, this acetyl CoA would go through the Citric Acid Cycle and onto the Electron Transport Chain, making energy that can be used by ALL of your cells. ( In a cell that is in the state of Ketosis (AKA Starvation), the acetyl CoA is not used for the citric acid cycle, but is instead used to make the ketone bodies. The ketone bodies then make their way to the brain. Not to mention, gluconeogenesis is also occurring (at depleted rates at this point …. you know, because you’re running out of those scary glycogens.) And remember, gluconeogenesis is also energetically expensive, along with the production of ketone bodies. So all in all, you are losing energy by resorting to Ketosis.
The main electrolytes affected by this process are sodium, potassium and magnesium. There are other electrolytes in your bloodstream, such as calcium and phosphorus, but their levels don’t change much on a keto diet. However, getting sodium, magnesium, and potassium in balance can help prevent against symptoms of keto flu and fix some of the side effects you may have long term. Hello Dr. Jockers, in the past few months I have been working on cleaning up my diet. Eating gluten & dairy free as much as possible. I started taking exogenous ketones at the same time. I got a juicer for Christmas and about 3 weeks ago I added celery juice ( 1st thing in the AM) and a carrot, apple, turmeric juice in the afternoon. Some days I am fine and other days I have sudden diarrhea within an hour of drinking the celery juice. I’ve read so much about the benefits of drinking it but am wondering if the ketones are reacting to it adversely? Please let me know your thoughts on this.

So one study looked at the long-term effects of a keto diet in obese patients and after 24 weeks, patients lost weight, reduced their total cholesterol, LDL, triglycerides and increased their HDL. Another study conducted on 132 obese patients found that the low carb (keto) group lost more weight than the low fat group while improving biomarkers like decreased triglycerides, improved insulin sensitivity, and decreased fasting glucose. This all was confirmed in a 2013 meta-analysis, 13 RCTs (1,569 participants) found that patients assigned to a very low carbohydrate diet resulted in greater weight loss compared to those assigned to a low-fat diet.
The truth is, I could have drank all the water in the world, and it wouldn’t have solved the problem, because the diet kept me from holding it. I was on the diet approximately six months when it started happening, with warning signs, in retrospect, a month before. When I wrote my story on a blog, I was contacted by a law firm who wanted to investigate my story, because they had a few other clients with similar experiences. I declined their services.

Here’s to exciting beginnings in cancer research: While we don’t have a lot of human studies to draw on, early findings suggest that the keto diet may have anti-tumor effects by reducing the total energy for tumors to thrive. We’ve also seen animal models report successful reductions in tumor growth, gastric cancer, and prostate cancer by using a ketogenic diet.
In a pilot study, five out of seven patients trialed a keto diet for 28 days and showed marked reduction in physical symptoms. Parkinson’s attacks our human nervous system, partially as a result of an abnormal accumulation of a protein called alpha-synuclein. Research suggests that a ketogenic diet may reduce the associated cognitive and motor symptoms.Obviously, we need more research here but its an exciting finding.
The ketogenic diet is a high-fat, adequate-protein, low-carbohydrate diet that in medicine is used primarily to treat difficult-to-control (refractory) epilepsy in children. The diet forces the body to burn fats rather than carbohydrates. Normally, the carbohydrates contained in food are converted into glucose, which is then transported around the body and is particularly important in fueling brain function. However, if little carbohydrate remains in the diet, the liver converts fat into fatty acids and ketone bodies. The ketone bodies pass into the brain and replace glucose as an energy source. An elevated level of ketone bodies in the blood, a state known as ketosis, leads to a reduction in the frequency of epileptic seizures.[1] Around half of children and young people with epilepsy who have tried some form of this diet saw the number of seizures drop by at least half, and the effect persists even after discontinuing the diet.[2] Some evidence indicates that adults with epilepsy may benefit from the diet, and that a less strict regimen, such as a modified Atkins diet, is similarly effective.[1] Potential side effects may include constipation, high cholesterol, growth slowing, acidosis, and kidney stones.[3]
The primary outcome, hemoglobin A1c, decreased from 7.5 ± 1.4% at baseline to 6.3 ± 1.0% at week 16 (p < 0.001), a 1.2% absolute decrease and a 16% relative decrease (Table ​(Table4).4). All but two participants (n = 19 or 90%) had a decrease in hemoglobin A1c (Figure ​(Figure1).1). The absolute decrease in hemoglobin A1c was at least 1.0% in 11 (52%) participants. The relative decrease in hemoglobin A1c from baseline was greater than 10% in 14 (67%) participants, and greater than 20% in 6 (29%) participants. In regression analyses, the change in hemoglobin A1c was not predicted by the change in body weight, waist circumference, or percent body fat at 16 weeks (all p > 0.05).
Type 1 diabetes causes the same blood sugar control issues as type 2 diabetes, but in an entirely different way. Type 1 diabetics cannot produce enough insulin or any insulin at all, which requires them to have insulin administered exogenously. On top of that, the perfect diet will not be able to reverse this disease as the ketogenic diet can for type 2 diabetes.

“If not done right, this diet can cause havoc on our bodies,” says Mancella. “As a society, we’ve turned to a culture of restriction and extremes in order to obtain unrealistic beauty and aesthetic standards without considering the long-term consequences. This diet doesn’t only affect metabolism, but also every other component of our bodies. By introducing potential vitamin and mineral deficiencies, we’re possibly trading the present moment for quality ones in the future.”
The backbone of a keto plan is its extraordinarily high fat content, making up 65 to 80 percent of calories daily. Protein—which can raise blood glucose, though not as much as carbohydrate does—makes up 15 to 25 percent of calories on the keto diet. And carbs are even more heavily restricted to just 5 to 15 percent of calories. That’s only about 20 to 50 grams a day (compared with the average 245 grams daily), or the amount in a small apple or a cup of cooked brown rice, respectively.
The alluring promise of the keto diet, potentially filled with as much bacon, butter, eggs, and avocado as you can eat, sounds like the grown-up version of scoring a golden ticket to Willy Wonka’s Chocolate Factory. I mean, who wouldn’t want to chow down on bacon and butter at every meal? Especially if you ended up dropping a few pounds along the way?
The keto diet also has an impact on our hormonal levels. Many studies have looked at whether the state of ketosis suppresses our appetite through the actions of leptin and ghrelin. A 2013 study found that after patients lost weight on a keto diet, our hunger hormone (ghrelin) was altered and suppressed. A systematic review also concluded that the state of ketosis appears to be a plausible explanation for the suppression of appetite. So this the keto diet may be good for dieters who can’t stand the discomfort of hunger. Finally, the keto diet also may have an impact on our stress hormone, cortisol. This was demonstrated in a Harvard study where the keto diet resulted in an increase in cortisol in individuals following a very low carb keto diet. High levels of cortisol is associated with insulin resistance, cardiovascular disease and may promote fat accumulation.
The keto diet dates back to the 1920s, explains Michelle Hyman, MS, RD, CDN a registered dietitian at Simple Solutions Weight Loss. "The original keto diet was designed for patients with forms of epilepsy that were resistant to standard treatments. The macronutrient breakdown was designed to mimic the fasting state — which seemed to help with relieving seizures — yet provide energy and nutritions to function," Hyman says.
As Mark Sisson puts it, doing a keto reset restores our “factory settings,” which is our flexibility to alternate between different types of fuels and stored fats for energy, depending on what’s available. This flexibility has allowed humans to thrive for millions of years because hunter-gatherers didn’t always have access to constant abundance and variety of foods that we have today.

What others (many of them) out there provide is just incomplete knowledge, claims, and advises that are not backed by enough evidence. By now, you've probably heard all about the ketogenic diet, the high-fat, low-carb eating style everyone's talking about. And while the benefits of keto are pretty well-known—weight loss and a suppressed appetite—the downsides are usually minimized by keto advocates. "The positive aspects of a ketogenic diet are spoken about much more than the drawbacks," says Alix Turoff, R.D., C.D.N., C.P.T., a dietitian and personal trainer.
so I won’t repeat it all here. The original study that claimed calcium supplements caused heart disease was widely publicized when it came out but unless you had a subscription to the British Medical Journal you wouldn’t be aware of the serious objections published in subsequent issues. In particular, Lappe & Heaney reported the complete opposite result – calcium supplementation REDUCED heart disease versus placebo.
Clinical trials that compare various diets’ outcomes among patients with mental health conditions are sorely lacking. For example, while currently there are 2822 clinical trials registered at Clincaltrials.gov for schizophrenia, none of them are examining the ketogenic diet’s impact on this debilitating chronic condition (three however, are examining gluten-free diets.) Likewise, there are NO ketogenic interventions among 1180 clinical trials for bipolar disorder, 2711 studies for anxiety, and 5370 for depression. (Although there are still a number of trials for these conditions that are looking at “low-fat healthy diets” or “Mediterranean diets” with plenty of fruits, grains and vegetables.)
But when you consume a high-carbohydrate diet, your brain often can't convert enough glutamate into GABA because it's using glutamate as an energy source. This imbalance can lead to neurotoxicity, and this impaired functioning could potentially lead to brain fog. For reasons not completely understood, ketosis seems to favor an increased production of GABA, which can lead to reductions in neurotoxicity and, therefore, brain fog.
"Most of the work in this field is still pre-clinical, meaning it's been conducted in animal models," Angela Poff, a research associate in the Department of Molecular Pharmacology and Physiology at the University of South Florida, told U.S. News & World Report. "It's been done in various cancer types, but most of the work has been done in brain cancer specifically. But there's very little clinical data all around. There's some case reports and very small preliminary clinical studies in small groups of patients, usually very late-stage patients with various types of cancers. So in the clinical realm, which is the most important in telling us whether this is going to be useful, we have a long way to go."

Other potential culprits, Dr. Ede notes, are grains and legumes, which not only interfere with absorption of brain-healthy nutrients but are also high in lectins and other factors that may pose risks to brain health. Cutting out refined carbohydrates, refined oils, grains and legumes through the low-carb high-fat (LCHF) or ketogenic diet may improve mental health, just as doing so can reverse diabetes and promote weight loss, she notes. 
Sleep enough – for most people at least seven hours per night on average – and keep stress under control. Sleep deprivation and stress hormones raise blood sugar levels, slowing ketosis and weight loss a bit. Plus they might make it harder to stick to a keto diet, and resist temptations. So while handling sleep and stress will not get you into ketosis on it’s own, it’s still worth thinking about.
Hello everyone. I was diagnosed as a diabetic in 2010. I followed the ADA diet while taking metformin and lnsulin and could never get by glucose readings below 135. Most mornings it was at 175 or higher. In mid 2017 I had to find a new doctor. I ran out of meds in Semtember of that year but could not find a doctor due to not accepting new patients and or my health insurance. I broke my foot at work on the last workday in December. Was instructed to stay off foot for 6 to 9 months . In the mean time I finally got to see my new family practitioner on March 28 2018. Represcribed meds and ordered blood work. A1c was ar 14.1. With my new glucose meter my readings were 375. Due to basically being bedridden while my foot heals I was concerned about diabetic complications an weight gain as I was already overweight. After doing research online I learned about the keto diet. I began the diet on 3/29 /2018 along with intermittent fasting. I weighed 265#. As of 4/26/2008 I am at 245#. My glucose readings have been on average 73 to 98 and a couple of times 111. I stopped all my medication about a week ago just to see if they would increase. They have not so far and I check 4 times daily. I sleep better and do not crave sweets. I feel full . The first two weeks were tough but now I can go 2-3 days without being hungry. I am looking forward to the results of my next blood test in June. This diet fits my circumstances and I do plan on to exercise when I am able to. I want to reach my weight to height ratio also. When I achieve this goal I may tweak my diet at that time but for right now that is what is working for me. I may never be able to eat some of the things I used to but considering the complications of diabetes it is one hell of incentive for willpower to stay on the diet.I will repost again after my next blood test or if there are any significant changes.
Yancy has seen similar results in his practice. “Carbohydrate intake is the main driver of blood sugar. So if you’re able to lower blood sugar by reducing carbohydrate intake, then you may be able to reduce diabetes medication,” he says. “We’ve seen people come off of hundreds of units of insulin just by changing the way they eat, and that can happen really quickly, in just a few weeks.”
Keto diets are great for losing weight. And everyone should be able to make their own decision AFTER being fairly apprised of the risks, which are too often glossed over. A balanced and honest discussion of the pros and cons, is what helps people make an informed decision, where they are aware and consenting of possible risks they may be exposing themselves to.

A huge concern with the keto diet is the maintenance and potential loss of muscle mass. Many people will just think: hey, dummy, then just eat more protein. However, some research has shown that even if your protein intake remains constant, a low carb diet may promote muscle loss. A study from the Netherlands confirmed these findings. In the study, participants were given three diets (high carb, moderate carb, low carb) and moderate protein. The study found that those following a low carb diet experienced increased muscle breakdown. This is because when we eat carbohydrates, we produce insulin which promotes muscle growth. This is why athletes depend on carbohydrates (along with protein) to fuel their performance. When we eat carbs, the insulin release “unlocks” our muscles to let the protein in so it can do its job at building our muscles. So, when we skip the carbs all together, muscle glycogen stores get depleted, we lose out on those muscle building opportunities. Forget about high intensity training. A depleted glycogen store also means our workouts will suffer because we just don’t have enough oil left in the tank. This was a again suggested in the recent review looking at many ketogenic studies. The studies found that there was greater lean body mass loss in the ketogenic groups compared to the other diets being studied.


In the mid-1990s, Hollywood producer Jim Abrahams, whose son's severe epilepsy was effectively controlled by the diet, created the Charlie Foundation to promote it. Publicity included an appearance on NBC's Dateline programme and ...First Do No Harm (1997), a made-for-television film starring Meryl Streep. The foundation sponsored a multicentre research study, the results of which—announced in 1996—marked the beginning of renewed scientific interest in the diet.[1]

"Many of the richest sources of fiber, like beans, fruit, and whole grains are restricted on the ketogenic diet," registered dietician Edwina Clark told Everyday Health. "As a result, ketogenic eaters miss out on the benefits of fiber-rich diet such as regular laxation and microbiome support. The microbiome has been implicated in everything from immune function to mental health."
For children, fill up an adult-sized water cup with each meal and request they finish it before they leave the table. Usually they will be thirsty and this is not an issue. If you are concerned that they are not drinking enough, stevia flavor enhancers can encourage this, carb-free.  Ice chips are popular in my house as well, and an additional way to get liquids into children.  Don’t force or encourage excessive water consumption, but rather give children the down-time to drink to thirst rather than rushing back to play, ignoring their thirst.
If you’re wondering about the difference between ketosis and diabetic ketoacidosis, you’re not alone. “The word ketone is scary for most people with type 1 diabetes because they relate ketones to diabetic ketoacidosis,” says Patti Urbanski, MEd, RD, CDE, a certified diabetes educator with St. Luke’s Hospital in Duluth, Minnesota. “But with the ketogenic diet, we’re talking about a much lower level of ketones.”
There are occasional instances of cardiomyopathies (heart muscle diseases, heart failure) and sudden cardiac death–The immediate causes are unclear, though at least some cases are due to severe selenium deficiency. The picture is muddied by the fact that some kids were fed large quantities of corn oil in past as a means of maintaining ketosis. Nonetheless, it suggests further uncertainties with prolonged ketosis.
Studies are emerging that ketogenic diets (in conjunction with other treatments) can either reverse progressive brain disorders or help repair the damage. These include traumatic brain injuries and neurodegenerative diseases like Alzheimer’s and Parkinson’s. The Wahl’s Protocol also utilizes this benefit of the ketogenic diet to help repair neurological damage from multiple sclerosis.
Researchers note that subjects taking between 40 to 90 units of insulin before the study were able to completely eliminate their insulin use while also improving blood sugar control! They also point out that this effect happens “immediately upon implementing the dietary changes” so people with type 2 diabetes need to monitor their blood sugar closely and likely adjust their medication dosages/needs with the help of their doctors.

Registered dietitian Julie Stefanski is based in York and has specialized in the ketogenic diet for more than a decade. She also advocates for anyone starting keto, especially individuals who have not seen a doctor in more than a year, to first see a physician and/or registered dietitian to have their lab work checked and monitored. Mild ketosis is a natural phenomenon that occurs in humans during fasting and lactation (19,20). Postexercise ketosis is a well-known phenomenon in mammals. Although most of the changes in the physiological parameters induced following exercise revert back to their normal values rapidly, the level of circulating ketone bodies increases for a few hours after muscular activity ceases (21). It has been found that in trained individuals, a low blood ketone level protects against the development of hypoglycemia during prolonged intermittent exercise (22). In addition, ketosis has a significant influence on suppressing hunger. Thus, a ketogenic diet is a good regulator of the body’s calorie intake and mimics the effect of starvation in the body.

"Keto is not a great long-term diet, as it is not a balanced diet," Nancy Rahnama, M.D., M.S., an internal medicine and bariatric specialist, told Reader's Digest. The low-carb, high-fat approach to the keto diet limits the types of foods you can have, and entire food groups are eliminated entirely. Beans, legumes, and whole grains are out, as are many fruits and vegetables. Many of these foods carry vitamins, minerals, and other nutrients you can’t get from any other source, and without them, you may start to experience nutritional deficiencies.
My pick for the healthiest meal plan for diabetes? My favorite is the Mediterranean Diet. It’s high in fiber, low in saturated fats and includes no processed foods which is the challenge for all of us at this point in history. If we could all eat like they do in Italy and Greece! Think of Sicily and the coasts of Greece where their diet consists of fresh fish, fresh fruits and vegetables, nuts, beans, seeds, olives and olive oil, lean meats such as chicken and pork, some eggs and little red meat. The American Heart Association recommends it as well as the American Diabetes Association as being one healthy diet choice for people with diabetes.

The keto diet is one of the hottest weight-loss strategies today. That’s not surprising—who wouldn’t want to lose weight fast without counting calories or feeling hungry? But it’s not without controversy: Yes, you can eat all the fatty foods you want, but the tradeoff is the extreme restriction of carbohydrates: bread, potatoes, even fruit. Keto dieters are advised to keep carbs to just 10 percent of their daily intake.


Early humans probably were on an LCHF diet. But when did humans become the “intelligent” species that they are? Can that be related to invention of agriculture? When humans began settling down on river banks to grow their crop, be it rice or wheat or maize, may be the rich alluvium, elevated mineral contents and higher glucose levels associated with grains might have given them increased brain activity leading to their cultural and intellectual development. My only worry is, this”new found” ketogenic diet shouldn’t push us back to stone age, though on a positive note, that might save our planet from anthropogenic destruction!

What is diabetic ketoacidosis? When a diabetic (usually a Type I diabetic, but sometimes this occurs in very late-stage, insulin-dependent, Type II diabetics) fails to receive enough insulin, they go into an effective state of starvation.  While they may have all the glucose in the world in their bloodstream, without insulin, they can’t get any into their cells.  Hence, they are effectively going into starvation.  The body does what it would do in anyone – it starts to make ketones out of fat and proteins.  Here’s the problem: the diabetic patient in this case can’t produce any insulin, so there is no feedback loop and they continue to produce more and more ketones without stopping.  By the time ketone levels (specifically, beta-hydroxybutyrate) approach 15 to 25 mM, the resulting pH imbalance leads to profound metabolic derangement and the patient is critically ill.
