I was a Corpsman (not a corpse-man as some recent somewhat fanatical president would say), and I can tell you many stories of Marines and Sailors who maintained restrictive diets (aka picky eaters). Most obvious was lack of sustaining energy (hypoglycemia) at mile 15 (with 80lbs of gear including a 6.5lb rifle and 200 rnds of ammo, etc.) and depletion of essential vitamins, electrolyte imbalance. They were always the first to collapse and have to hear me scold “see I told you so.” An IV of D5W usually does the trick (D is for dextrose, OMG!)

By now, you've probably heard all about the ketogenic diet, the high-fat, low-carb eating style everyone's talking about. And while the benefits of keto are pretty well-known—weight loss and a suppressed appetite—the downsides are usually minimized by keto advocates. "The positive aspects of a ketogenic diet are spoken about much more than the drawbacks," says Alix Turoff, R.D., C.D.N., C.P.T., a dietitian and personal trainer.

Following a ketogenic diet puts your body into a state of “ketosis,” which is a metabolic state that occurs when most of the body’s energy comes from ketone bodies in the blood, rather than from glucose from carbohydrate foods (like grains, all sources of sugar or fruit, for example). This is in contrast to a glycolytic state, where blood glucose (sugar) provides most of the body’s fuel (or energy).
A ketogenic diet is very low in carbs, high in fat and moderate in protein. For a weight stable person, no more than 5% of calories should come from carbs, 70-80% from fat and 20-30% from protein. With this macronutrient-distribution, individuals do not rely on glucose as a major source of energy but use fat for fuel instead. Fat is used to produce ketone bodies, which are used for energy or to regulate certain genes. Two ketone bodies circulate in in your blood: Acetoacetate (AcAc) and Beta-hydroxybutyrate (BHB). Wait, what?! How could a diet rich in meat, butter, and cheese do anything but increase the chances of a heart attack? Well, the tables might be turning in defense of fat. While it’s still unclear how the keto diet impacts heart health long-term, especially for those predisposed to heart disease, research has found that the keto diet may help improve triglyceride, HDL, and LDL levels, and improve overall cardiovascular risk factors. Pass the cheese, please!

This general “muscle wasting” assertion often comes from trainers and dietitians who really have not studied the science on muscle preservation. They will tell you that the brain requires at least 100 grams of carb per day and if you don’t get those carbs in the diet, your body will break down your muscles to get it. This is true when one’s diet is high carb, and no ketone bodies are available as an alternative source of brain fuel.

In a comprehensive 2016 paper, Bread and Other Edible Agents of Mental Disease,  the authors note that “the evidence is overpowering” for an association between wheat products and mental illness. While not everyone will see their mental health symptoms resolve, they recommend a wheat-free diet as a trial for anyone with mental health issues. The popular book Wheat Belly by Dr. William Davis, makes similar recommendations.

What the doctors never tell you is that you could also just eat completely different and take none of this crap. So that’s what I did. I stopped all of them all at once. Sure I don’t recommend that, but this damn disease and my disgusting visceral fat filled stomach ruined every aspect of my life-professional, relationships, sexual, mental. I am now 41 living back at home having to start from the bottom. I don’t want what the book recommends. I want every disgusting piece of fat off this body so I can excel again. If staying in ketosis every moment for the rest of my life will get me there then that’s what I’m going to do.
Also, to the author I do appreciate you stating you are bias up front, but I do get to indulge with some fruit/berries when I want. You dont give up everything forever but you learn to fit them into your macros. I have learned to make Keto ice cream and fat bombs if the urge comes along. I have learned to take keto friendly foods along to potlucks that everyone loves. Keto can be a way to follow forever but everyone has their own needs for their bodies. I am new to this but finding it easier and easier to remain keto
You sound weak I have given up all carbohydrates and fast acting insulin and had no issue with it ( real food is so delicious and satisfying)… and I don’t miss bread pasta and sugar at all… your mind is poisoned. It also has an added side effect , I now have no fat on my body. You uneducated money grubbing doctors and the sugar industry are the real problem here. My endocrinologist didn’t even bother to tell me about nutritional ketosis at all. I eat 80 to 85% good saturated fats out the 4000 calories I consume each day… not that difficult getting rid of crap sugar and carbs… these “doctors “ are lying to us and no one cares! It’s really disgusting …. I have no problem living without sugar…. and no studies done at all.. what the hell are these doctors doing, it seems pretty obvious to me$$$$.

The good news is, if you’re a generally healthy adult, you can probably tolerate the keto diet. “It’s likely appropriate and safe for almost everyone,” says Robert Santos-Prowse, a Clinical Dietitian and the author of The Ketogenic Mediterranean Diet and The Cyclical Ketogenic Diet. “Ketosis is a natural metabolic state that everyone has spent some time in at some point in their lives. That makes a lot of sense. Keeping up insulin pathways when you aren’t eating carbs would be like keeping the lights on when it’s daytime outside — it’s a waste of energy. You aren’t using insulin on keto, so your body probably downregulates your insulin pathways. As a refresher, insulin is a hormone produced by your pancreas that tells your cells to absorb glucose to use as fuel. When you eat carbs, insulin production begins. In the absence of carbs, there’s less need for insulin.

Doctors can measure levels of inflammation in the body using blood tests for high-sensitivity C-reactive proteins (hsCRP) and white blood cell (WBC) counts. What others (many of them) out there provide is just incomplete knowledge, claims, and advises that are not backed by enough evidence. And they do it all for the sake of pioneering in the debate/argument. Just because someone else is wrong about something, doesn’t automatically make you right. I have spent the complete day searching for Keto, learning about it, the risks, and how to implement it. And my search ended right here on this article. The kidneys play an important role in metabolizing protein, and it’s possible that eating too much of the nutrient can have a negative impact on kidney function. While ketogenic diets are supposed to be much higher in fat than they are in protein, many keto eaters make the mistake of loading up on lots of meat, Mancinelli says. The result? You could end up eating way more protein than you actually need.
If you’re a generally healthy adult who’s looking for a new way to eat, it’s probably fine for you to try the diet. Just think carefully about your motives, intentions, and long-term plans. Any diet you follow for a short period of time might garner some initial results, but if you’re not intentional about switching to a sustainable, long-term healthy eating pattern, the benefits you see while going keto are likely to be short-lived. Be honest with yourself about whether or not you have the self-control and desire to follow such a restrictive eating pattern. If you don’t, that’s OK. The best diets are those you can legitimately sustain for a lifetime. Restriction isn’t always the best answer.
The only difficulty with some of these studies is that they tend to have small sample sizes, like this one that only has five cyclist participants and the data was largely skewed by the fact that only ONE cyclist experienced a large enhancement of exercise capacity after the keto diet. Their studies also tend to be short term. Back in 2014, Phinney and scientist Tim Noakes wrote an editorial that stated that in the past 31 years, there have only been a handful of studies measuring sports performance and low carb diets. Out of a total of 11, only 3 found exercise improvements.

Turoff doesn't think the keto diet should be a go-to solution for weight loss, and she's not alone in this opinion. Many of the dietitians Shape spoke to for this story had similar thoughts on the keto diet, which is why many of them strongly encourage anyone who is thinking about trying a ketogenic diet to chat with a registered dietitian first. 2. Gluconeogenesis occurs due to hunger, when your blood glucose levels are low. This process is when your liver breaks down glycogens (stored carbs) and turns them into glucose. This is a very ENERGY EXPENSIVE process. (You burn 3x the amount of ATP than you do during glycolysis and the branching of glycolysis, which is the process that turns glucose into things like ATP, CO2, RNA, NADPH, etc.) So yeah, gluconeogenesis does exist, but it’s to help keep us alive and keep glucose flowing to the brain.

Researchers believe that the ketogenic diet can also help patients with schizophrenia to normalize the pathophysiological processes that are causing symptoms like delusions, hallucinations, lack of restraint and unpredictable behavior. One study found that the keto diet lead to elevated concentrations of kynurenic acid (KYNA) in the hippocampus and striatum, which promotes neuroactive activity. Some studies even point to the elimination of gluten as a possible reason for improved symptoms, as researchers observed that patients with schizophrenia tended to eat more carbohydrates immediately before a psychotic episode. Weight loss is the primary reason my patients use the ketogenic diet. Previous research shows good evidence of a faster weight loss when patients go on a ketogenic or very low carbohydrate diet compared to participants on a more traditional low-fat diet, or even a Mediterranean diet. However, that difference in weight loss seems to disappear over time.

The side effects of extreme low-carb diets are still a mystery. "The first major drawback is the fact that we really don't have any long-term research about how keto followers fare 10, 20, 30 years down the line," Turoff says. "What will the results look like? Will they regain the weight? Will they suffer health consequences? We also thought smoking and diet pills were a good idea, until long-term studies showed us that we were wrong."


My point here is that the warnings about the ketogenic principles are well taken and well documented. My concern is implications that this is a fad. I don’t use the word diet with my patients and I’m concerned that the principles behind the label and the real results that these readers have commented on might get minimized. I have found it best to encourage patients to read authors like: Stephen Phinney, Jeff Volek, Patricia Daly, and Charles Gant and the be partners with their doctors and check blood work as they move along. I am not for or against the article. If ketogenic principles offer people enduring, satisfying, and cohesive change then why not read about its potential and flexilbity?

The keto diet is an extremely effective way to lose weight over a short period of time—even better, according to some research, than low-calorie and low-fat diets. There a few reasons for this: When you’re in ketosis, your body stores less fat. Dieters feel fuller for longer, partly because of the rich food they’re eating, and partly because ketosis changes your hunger hormone levels.

Vegan ketogenic diet or vegetarian diet: Yes, both are possible. Instead of animal products, plenty of low-carb, nutrient-dense vegan and/or vegetarian foods are included. Nuts, seeds, low-carb fruits and veggies, leafy greens, healthy fats and fermented foods are all excellent choices on a plant-based keto diet. There’s also a similar plan called ketotarian, which combines keto with vegetarian, vegan and/or pescatarian diets for supposedly greater health benefits.
Essentially, the keto diet for beginners works by “tricking” the body into acting as if its fasting (while reaping intermittent fasting benefits), through a strict elimination of glucose that is found in carbohydrate foods. Today the standard keto diet goes by several different names, including the “low-carbohydrate” or “very-low-carbohydrate ketogenic diet”(LCKD or VLCKD for short).
I had type 1 diabetes for 30+ yrs; CKD/ESRD x8 yrs and did peritoneal dialysis for 5 yrs before getting a kidney pancreas transplant nearly 5 years ago. Then had a below knee amputation and a rather sedentry life since-need to lose weight. Is this a safe diet for me? I am terrified of ketosis (DKA nearly killed me when i was 12 yrs old) so anything messing with stuff like that really makes me nervous. TIA!
Additionally, you may find it difficult to eat adequate calories on a daily basis because fat is so filling. Some people even opt to fast for several hours each day. While this may help with weight loss, your body needs those calories for proper maintenance. Without them, it may turn to burning muscle for energy, and that can accelerate muscle loss.
