In a 14-day inpatient feeding study, 10 participants with type 2 diabetes experienced improvements in hemoglobin A1c and insulin sensitivity as measured by the euglycemic hyperinsulinemic clamp method [8]. Hemoglobin A1c also improved in an outpatient study of 16 participants who followed a 20% carbohydrate diet for 24 weeks [9].
Ketogenic diet for diabetics is a highly controversial topic, but we will break down everything here for you! ME: It helps by removing carbs and forcing the body to rely on fat for fuel. This sort of primes the body for using stored body fat for fuel. It also gets hormones under control, especially hunger hormones like leptin and grehlin. It works because it is a lifestyle, not a fad diet. People can stick with it because they feel so good, aren’t hungry and don’t have cravings anymore.

First off, make sure that you’re eating real and organic foods. I started the ketogenic diet because I am a diabetic and refuse to take the medications that Drs advise because I’ve had bad reactions to them. I have lost about 20 pounds and the blood glucose is better controlled. I am also sleeping well most of the time. I did have the diarrhea but that has worked itself out. The one problem I am having is low blood pressure. I awoke dizzy one night and decided to check it and it was 73/48. I add himalayan sea salt to the water as recommended but sometimes still have a problem. A friend recommended using cream of tartar but what I have read said that it lowers blood pressure. Would I have to worry about low blood pressure if I added this to my water???

It is not for the faint of heart and yes I am writing from experience. Of course I have tried it! Hasn’t everybody in America at some point who has wanted to lose weight? Does it work you ask? Of course it does! The problem is how long can you keep it up?

A diet low in fruits and vegetables can put you at risk for certain vitamin and mineral deficiencies. So stepping away from our high carb diet- I am sorry to say that we eat more carbs since the 70s with most of it processed and we now use high fructose corn syrup to sweeten products and we have a wide spread childhood obesity problem. The good news here is no! All the evidence points to the fact that a low carbohydrate diet actually does lower blood glucose and A1c levels and does contribute to weight loss. The problem is we do not yet have enough large studies, over enough sustained years to support evidence that people with diabetes can remain on a highly restrictive Ketogenic Diet for the rest of their life and also not have other consequences to their health.

When our thyroid hormone levels are low, LDL receptors will be less active, leading to high cholesterol levels and an increased risk of heart disease. If you have a history of hypothyroid issues, you may be struggling with unhealthy cholesterol levels — and the keto diet can make them even worse. However, for those of you who are being treated for your hypothyroid condition or who have an autoimmune thyroid condition, you may be able to follow the keto diet without any problems. In fact, many keto dieters with autoimmune thyroid conditions have found that the keto way of eating improves their quality of life more than any other diet.

From baseline to week 16, the mean body weight decreased significantly from 131.4 ± 18.3 kg to 122.7 ± 18.9 kg, BMI decreased from 42.2 ± 5.8 kg/m2 to 39.4 ± 6.0 kg/m2, and waist circumference from 130.0 ± 10.5 cm to 123.3 ± 11.3 cm (Table ​(Table3).3). The percent change in body weight was -6.6%. The mean percent body fat decreased from 40.4 ± 5.8% to 37.0 ± 6.0%. Systolic and diastolic blood pressures did not change significantly over the 16 weeks. The mean heart rate decreased from 81.2 ± 12.9 beats per minute to 74.6 ± 14.0 beats per minute (p = 0.01).

Mike, that’s exactly right! With T2, we no longer have the option of eating carbs, sugar and all the good stuff. Why can’t dieticians and the ADA recognize that and quit trying to shove all those carbs down our throats? I don’t get it… I seriously don’t. And I think the author of this article would do an about face is she actually had diabetes. It’s amazing the amount of people who claim to be experts that seriously don’t get it!! It I had Celiac Disease, I couldn’t eat gluten… at all. Why is the same not recognized for diabetics? Our meters show us when we are eating too many carbs. Its VERY clear as the number goes very high. What do the professionals not get about that? It’s been the most amazing thing about this whole process for me and I just can’t believe how biased people are against a very low carb diet for managing diabetes. You think that because people can’t maintain that kind of diet for long term makes it OK to go ahead and be against it? Did it ever occur to any of the professionals that by recommending a low carb diet it might actually encourage people to maintain it? Instead, you are giving them excuses and reasons to eat way too many carbs!! Last August 2016 I was diagnosed with T2, with an A1C of 12.7. My last blood test showed an A1C of 6.2 (July 2017) and I had reduced some of the meds I was originally on. I am still working on lowering my numbers. The whole process has been a slow progression to keto and I had to stumble on the whole thing myself through my own research. I tried vegan at first and quickly realized that I was eating too many carbs. Then I went low carb but knew I could do better. When I tried the Keto diet, my numbers went much lower. You get over the sweet addictions. You get over the bread addictions and you find suitable substitutions. You do what you have to do. But by not recommending an ultra low carb diet simply because you don’t think people can do it is ridiculous! It is basically telling people that they can’t possibly manage their own lives… they can’t possibly make their own, good choices. And then, because you are the authority, you are giving them reasons to not even try. You defeat them before they even begin. It just amazes me!

I’ve been following a ketogenic based “Way of Eating” or almost a year now, and I’ve never felt better. Fats are the main source of calories with it—NOT protein. Fats and proteins ARE essential for the body. Carbohydrates are not. The 20g of carbs are for net carbs, NOT total carbs. This means you can have a great deal of fiber from very nutrient filled veggies that don’t spike your blood sugar.
Once the body gets used to manufacturing ketones as the main energy substrate, the body actually has more energy than it previously had, and you won't have to be fighting through all those low-blood-sugar crashes your high-carb meals previously gave you. Additionally, hydration should be an area of high priority, especially before, during, and after exercise.
It's really the trans fats that you'll want to avoid completely. "Trans fats are highly processed, and heated to a point where they create free radicals in the body, which increase your risk of cancer.," she says, "Trans fats are found in hydrogenated oils (be on the lookout for this form of fat listed on the ingredients), and these are mostly used in the preparation of processed crackers and packaged foods."
These carbohydrate recommendations are not PER DAY, they are per meal. This is the major difference between Ketogenic diets and a consistent, low carbohydrate diet CDEs advocate for. 30-45 carbs per meal is considered a low carb diet. Eating this in the form of a Mediterranean diet is what I recommend and what my article outlined. In a perfect world, we try to have people steer clear of anything artificial or processed when possible. Just eat fresh foods, like our grandparents did, who never had these types of health problems.
Well… what are many people on a Keto diet trying to do? Lose weight right?  I would guess that prior to discovering the Keto diet many of these people were caught in the trap of low fat dieting.  This lead to months or years of low fat intake and consequentially low gallbladder activity.  The gallbladder wasn’t needed to digest fat and so it sat idle and stones were more likely to form.  Once they made the shift to a ketogenic lifestyle and their fat consumption increased upwards of 1000%, the gallbladder kicked into high gear.  If the previous period of low fat dieting had caused stone formation they are going to have to deal with flushing those stones out now while on a high fat Keto diet.
