However, the complete opposite occurred, practitioners began to lose excess body fat, feel healthier, and in some cases, issues of diabetes even began to subside. Nutritionists began to study the effect of a fat-based diet and its impact on human health, years later; the modified ketogenic diet emerged from the research and had been used with success in modern society.
When you restrict carbohydrates, the body begins to process electrolytes in a different way. This is because, under conditions of low insulin, the kidneys excrete more sodium. Since there is a delicate balance between sodium and other electrolytes in the body, this increase in sodium excretion can have a knock-on effect and disrupt other electrolytes as well.
Although various short-term studies examining the effect of a ketogenic diet in reducing the weight of obese patients have been carried out (10), its long-term effects in obese subjects are not known (15). Good article. A friend at work is stating this diet and of course being new at it, he is quite the Zealot. I have been into nutrition and exercise for over 40 years (57 years old now). I have tried s few different eating philosophies (I never use the word the word diet. Negative connotation and dieting isn’t good the way most practice it.). Eating Keto style and the logic behind it aren’t too different than some others. And you pointed out many of the misconceptions about cholesterol and triglycerides ect. For about the last 20 plus years I have pretty much followed a “Zone” type diet. 40% carbs, 30% fat, 30% protein. And I always get weird looks at the 30% fat part. Well, I’m 57 with 7% body fat. Not bad for and old guy. I work out and I supplement with protein as I lift weights. It’s worked well. The ratios may be different, but the emphasis on vegetable carbs and good fats are the key to either way to eat. There were only a couple things that kinda struck me as something that made me go hmmmmm. Using the Inuit people isn’t a good example really. They have evolved some different physical features over hundreds of years than we have. And there is no evidence if ketosis occurring in examination. And somehow when you mention eating large amounts of animal fat, just hit me as counterintuitive. It’s a much different fat than an avocado. Finally, and this is just me, I love fruit. True it’s probably less efficient, but its good, sweet, and beats the hell outta donuts for your health. It fulfills the reward need many of us have. I also feel if God put it on the earth naturally, it’s got a place in our food source. I’ve had a philosophy about excersise that’s served me well. What’s the best exercise? The one that you’ll do regularly. Get up and move and find out what works for your own body. It applies to good too. If you feel deprived or for various reasons can’t stay on the supposed best food program, what good is it? But if you stay with more natural and Balanced foods that usually leads to more energy then more and regular excerise. Check in. It looks like it’s been 2 years since you wrote this. Diabetes is actually a metabolic problem. It’s characterized by the hormone insulin no longer exerting its important actions appropriately. Your cells and tissues don’t ‘hear it knocking’ so to speak. So you produce more and more insulin, which eventually fatigues your pancreas. This deluge of insulin (hyperinsulinemia) you now find yourself bathing in also causes your fat stores to release fat inappropriately. This ultimately swamps your liver with excess fat (energy). To reduce the excess energy, your liver handles starts frantically producing and pumping out lots of blood glucose. This task (gluconeogenesis) is costly and momentarily solves your liver’s crisis of energy excess. The longer-term cost however are serious consequences like high blood sugars, unstable energy levels, damaged tissues (via glycation and ROS), neurodegeneration and worse body composition (with fat usually stored in the wrong places), etc.
It is completely wrong to discuss “average lifespan”. The average lifespans of pre-industrial peoples is heavily reduced by infant and early childhood mortality, which has nothing to do with lack of fresh fruit. Once you remove this bias in the numbers, pre-industrials can have lifespans almost as long as ours. And usually without many of the degenerative diseases that bother our middle and old ages.

As someone who is prediabetic, Keto has been a godsend! I thought I was just a glutton, but now I’m certain I am actually insulin resistant. I am on Keto and can manage half a cup of blackberries without coming out of ketosis, but almost any other carb/sugar and I become a mindless food consuming zombie. I literally feel like I have zero control over my ability to stop eating. Also before trying Keto, if I was not able to eat constantly throughout the day I became grumpy and tired. Like none tired where it felt like I had lead running through my veins. On Keto, I can go a good 5-7 hours without eating and I don’t feel off at all. I am hoping to lose weight, but mostly I do not want to become diabetic and my problem is that I am insulin resistant. I do plan eventually to eat more black beans, vegetables and fruits again. Holy cow Batman i started keto diet on the 9th of January within 4 days I was no longer on bydureon (weekly insulin) levemir 60units 2x daily and no more Novolog 25 units3x daily all that and metformin 10002x and this all didn’t have my sugars under control amazingly keto diet got sugars in check within four days I’ve been on diet now for 15 days and have honestly never fealt better I’m still green on diet and have had some issues while my body is healing itself (keto flu,low electrolytes very sluggish) .. As far as the Ketogenic Diet is concerned, I think I am going to have to make a full about-face. As you said, for some people like yourself, you were unable to eat in moderation and feel it is due to your genetic makeup. This is probably true…to add insult to injury, in addition to genetics, it could also be an inflammatory process going on due to the sugary drinks and processed foods we are eating causing obesity and a host of other issues.
That makes a lot of sense. Keeping up insulin pathways when you aren’t eating carbs would be like keeping the lights on when it’s daytime outside — it’s a waste of energy. You aren’t using insulin on keto, so your body probably downregulates your insulin pathways. As a refresher, insulin is a hormone produced by your pancreas that tells your cells to absorb glucose to use as fuel. When you eat carbs, insulin production begins. In the absence of carbs, there’s less need for insulin.
In a pilot study, five out of seven patients trialed a keto diet for 28 days and showed marked reduction in physical symptoms. Parkinson’s attacks our human nervous system, partially as a result of an abnormal accumulation of a protein called alpha-synuclein. The trick to reaping the benefits of the keto diet is to stay in ketosis, which means keeping your carbs at 5% or less of your calories. The 5% can fall anywhere between 20-50 grams a day. Familial Hypercholesterolemia. Familial hypercholesterolemia is a condition where one or more of the genes for the LDL receptor are defective, making it more difficult for them to clear cholesterol from the blood. If a person with familial hypercholesterolemia is then put on a high-fat diet, they will become even more vulnerable to heart disease as the increased saturated fat and cholesterol content of the diet provokes higher cholesterol levels. To counteract these genetic vulnerabilities, it is may be best to follow a low to moderate fat diet with plenty of whole foods, fiber, monounsaturated fats, polyunsaturated fats (especially omega 3s), and limited saturated fats. This, as well as a lifestyle that prioritizes physical activity, stress relief practices, and plenty of sleep, should keep their cholesterol levels under control.

Thanks for this article. I just started a Keto diet so found it appropriate to my current lifestyle. Though I don’t believe your bottom line is strong enough since you simply stating that the diet is “hard to follow” and food is “notoriously unhealthy” without evidence going deeper into why those “notoriously unhealthy” foods are worse than keeping carbohydrate-heavy food that are addictive and give the body a quick sugar high for energy. I believe “hard to follow” is your opinion only, since acceptable Keto foods are found at all restaurants easily and also all grocery stores. All the foods you mention: “rich in very colorful fruits and vegetables, lean meats, fish, whole grains, nuts, seeds, olive oil, and lots of water” are all Keto-friendly. Many people have been on a Keto-diet for years. A healthy lifestyle is a healthy mindset change and making right choices – it’s not going to be easy.
The keto diet is a low-carb, high-fat dietary regimen which has been linked to improvements in insulin sensitivity and higher rates of weight loss — both positive factors in managing type 2 diabetes. Lowering carb intake induces a metabolic state known as ketosis, through which the body produces ketones which burn fat — rather than carbohydrates — for energy.
But the question remains, can you keep it off? Aside from the fact that it’s a pretty hard diet to keep up (ah, no birthday cake!), one large meta-analysis found that while low-carb dieters tended to lose more weight than low-fat dieters at first, the differences disappeared by the one-year mark. This may be because cutting carbs tends to reduce bloating and water weight, which may level out over time. But the bottom line on weight loss? Calorie restriction is calorie restriction. You just have to find what works best for you, and slashing carbs for fat might or might not work.
One notable clinical trial of the ketogenic diet for schizophrenia occurred in 1965. Back then, one of the authors noted that in some of his schizophrenic patients a carbohydrate binge preceded eruption of their hallucinations and paranoia. The study put 10 women with schizophrenia on a ketogenic diet for two weeks. The diet was added to their standard treatment of medication and ECT (electroconvulsive therapy) and resulted in a significant decrease in symptoms. A week after the women resumed a standard diet, symptoms returned. Despite this preliminary, positive outcome, few researchers in the intervening 50 years have investigated the promising potential of the ketogenic diet in schizophrenia.
In another study on women, researchers confirmed that the ketogenic diet resulted in favorable changes in LDL particles consistent with lower cardiovascular disease risk. However, the total LDL cholesterol did not change. This is why it is important to test the levels of different LDL particles. Circulating ketone bodies make the blood too acidic, which will draw calcium from the bones as a buffer response. While there are relatively few studies on long-term (more than 6 months) effects of a non-therapeutic keto diet, studies of children on the diet show high calcium levels in the blood, increased bone demineralization and increased risk of kidney stones.
Longer-term studies conducted on animals have shown the KD may be associated with some adverse events. For example, in rodent studies, some will develop nonalcoholic fatty liver disease (defined as liver damage that is not due to excess consumption of alcohol, viral or autoimmune causes, and iron overload ) and insulin resistance when put on the keto diet long-term. Other studies suggest that some individuals may be predisposed to heart-related problems if they eat a very high-fat diet for an extended period of time.
The ketogenic diet is a high-fat, adequate-protein, low-carbohydrate diet that in medicine is used primarily to treat difficult-to-control (refractory) epilepsy in children. The diet forces the body to burn fats rather than carbohydrates. Normally, the carbohydrates contained in food are converted into glucose, which is then transported around the body and is particularly important in fueling brain function. However, if little carbohydrate remains in the diet, the liver converts fat into fatty acids and ketone bodies. The ketone bodies pass into the brain and replace glucose as an energy source. An elevated level of ketone bodies in the blood, a state known as ketosis, leads to a reduction in the frequency of epileptic seizures. Around half of children and young people with epilepsy who have tried some form of this diet saw the number of seizures drop by at least half, and the effect persists even after discontinuing the diet. Some evidence indicates that adults with epilepsy may benefit from the diet, and that a less strict regimen, such as a modified Atkins diet, is similarly effective. Potential side effects may include constipation, high cholesterol, growth slowing, acidosis, and kidney stones.
For me, I chose to become a CDE because I had worked as a Registered Nurse first on a medical unit, then in a Medical ICU. These are the units where most of the people with diabetes are, so I had seen every horrific complication that diabetes can cause. When I had a chance to become a Diabetes Educator…to actually help PREVENT some of those horrible complications…I jumped at the chance!
On the flip side, hypos can be an issue, especially early on…and if you treat them too aggressively, they could knock you out of ketosis. I remember my first 3 weeks on keto, my CGM trend line hugged a blood sugar of around 80. It was glorious, but I had to reduce insulin substantially through trial and error and felt like I was low every five seconds.

You lose weight temporarily because “if you’re not hungry every five minutes and can work on your willpower,” you won’t eat as much, Nisevich Bede says. But while some research is promising — one study published in October 2013 in the British Journal of Nutrition found that the keto diet led to greater weight loss than a low-fat diet, for example (4) — there is a lack of long-term research (greater than two years) that suggests a highly restrictive diet like keto is superior for weight loss than others, and it’s certainly not right for everyone.
Early humans probably were on an LCHF diet. But when did humans become the “intelligent” species that they are? Can that be related to invention of agriculture? When humans began settling down on river banks to grow their crop, be it rice or wheat or maize, may be the rich alluvium, elevated mineral contents and higher glucose levels associated with grains might have given them increased brain activity leading to their cultural and intellectual development. My only worry is, this”new found” ketogenic diet shouldn’t push us back to stone age, though on a positive note, that might save our planet from anthropogenic destruction!
The ketogenic diet may seem like the Jekyll to the Hyde-like low-fat craze of the 1990s. When the body is first deprived of carbohydrates, usually felt at around 50 grams per day or less, the body starts with gluconeogenesis which is the body using stored glucose (glycogen) from the liver and muscles for energy. When the stored glucose can no longer keep up with energy demands, which will happen because there’s limited storage of glucose, the body turns to using ketone bodies for energy.
