The ketogenic diet is a high-fat, adequate-protein, low-carbohydrate diet that in medicine is used primarily to treat difficult-to-control (refractory) epilepsy in children. The diet forces the body to burn fats rather than carbohydrates. Normally, the carbohydrates contained in food are converted into glucose, which is then transported around the body and is particularly important in fueling brain function. However, if little carbohydrate remains in the diet, the liver converts fat into fatty acids and ketone bodies. The ketone bodies pass into the brain and replace glucose as an energy source. An elevated level of ketone bodies in the blood, a state known as ketosis, leads to a reduction in the frequency of epileptic seizures.[1] Around half of children and young people with epilepsy who have tried some form of this diet saw the number of seizures drop by at least half, and the effect persists even after discontinuing the diet.[2] Some evidence indicates that adults with epilepsy may benefit from the diet, and that a less strict regimen, such as a modified Atkins diet, is similarly effective.[1] Potential side effects may include constipation, high cholesterol, growth slowing, acidosis, and kidney stones.[3]
One way to find out if the keto diet is not be ideal for your cardiovascular health is by checking if your total-to-HDL cholesterol ratio is above 4 and/or your LDL-P remains high or increases after starting the keto diet. If this is the case for you, then you may fare better with a low to moderate fat diet with plenty of whole foods, fiber, monounsaturated fats, polyunsaturated fats (especially omega 3s), and limited saturated fats.

First – let’s admit that there are several different types of diets that produce dramatic improvements in weight loss and diabetes. The vegan diet is one of them (and one which also reduces risk in most other diseases better than the others) – but it is by unquestionably by far the very best diet for the environment and the survival of the planet. High protein (high meat and/or dairy) diets are absolutely TERRIBLE for the environment and are not sustainable in any way. A vegetable diet will END world hunger because we DO have enough earth to grow enough vegetables for everyone and we definitely do NOT have enough earth for meat eaters even at current levels.

I am sorry you had this experience. I feel that this educator was not giving you good advice. All my women who want to lose weight are recommended to consume 30 grams of good carbohydrates at each meal, and 15 at each snack. If you were not trying to lose weight, I would have recommended 45. I find this is all it usually takes to begin to lose some weight as you start to get active. Patients set their own goals with motivational help from their Certified Diabetes Educator. Our intent is never to insult, and you should not have gone through that. It sounds that you have now found the right path. There are many CDEs who could help you, so see what tools and motivation others may offer. I wouldn’t let one bad apple spoil the whole bunch. Many CDEs are also diabetic.
Another potential benefit of the ketogenic diet is that it is thought to improve insulin sensitivity5,6. Prediabetics and diabetics are often referred to as “insulin resistant” because their muscle, fat, and liver tissues do not respond properly to insulin. When the pancreas can’t meet the body’s need for insulin, excess glucose builds up in the bloodstream, contributing to diabetes and other medical conditions7.

“If someone with diabetes is [taking insulin or oral type 2 meds in the sulfonylurea or meglitinide class and is] following this diet, they need to know that their blood sugar can drop really quickly, so it’s critical that they check it more frequently,” says Toby Smithson, MS, RDN, CDE, author of Diabetes Meal Planning & Nutrition for Dummies. “Don’t wait for it to happen. Meet with your doctor or diabetes educator in advance so that you can troubleshoot exactly what to do if your blood sugar drops.” If it’s an infrequent occurrence, you may be advised to treat with fast-acting glucose. Truly Dr. Colbert is also a reputable source for more thorough science and modification of previous Keto extremes. The statements about Keto diets with dairy everyday are not true…I am a 66 year old professional (University trained) and have found medical people often very unknowledgable or partially knowledgeable which may be worse. Obesity Compared with those on a typical low-calorie diet, obese individuals on a very-low-calorie ketogenic diet lost more weight and inflammatory visceral (belly) fat in one study, published in December 2016 in the journal Endocrine. (9) It may also help preserve lean body mass during weight loss, according to an article published in February 2018 in the journal Nutrition & Metabolism. (10)
Ketoacidosis, on the other hand, is a complication of poorly managed diabetes. It results in uncontrolled production of ketone bodies that can change the pH of the blood, which can prove fatal. Ketoacidosis is more common with Type 1 diabetes rather than Type 2. It’s also important to note that the keto diet cannot cause ketoacidosis – but rather puts the body into the beneficial state of ketosis.

One study from 2005 followed 22 people with Type 1 Diabetes for 12 months. The difference here however is they consumed 70-90 grams of carbohydrates per day versus the restrictive less than 20 grams per day on the Ketogenic Diet. Remember my motto? Moderation is the key! The results were positive; less hypoglycemia, insulin requirements were reduced and their A1c dropped from 7.5% to 6.4%.
The only evidence I could find for the long-term safety of a keto diet is from one 12 month trial. As a result of the study, the researchers found that overweight adults with elevated A1c that followed the keto diet experienced more significant reductions in A1c, lost more weight, and decreased their medications more than those instructed to follow a moderate-carbohydrate, calorie-restricted, low-fat diet.
Participants returned every other week for 16 weeks for further diet counseling and medication adjustment. When a participant neared half the weight loss goal or experienced cravings, he or she was advised to increase carbohydrate intake by approximately 5 g per day each week as long as weight loss continued. Participants could choose 5 g carbohydrate portions from one of the following foods each week: salad vegetables, low-carbohydrate vegetables, hard or soft cheese, nuts, or low-carbohydrate snacks. Diabetes medication adjustment was based on twice daily glucometer readings and hypoglycemic episodes, while diuretic and other anti-hypertensive medication adjustments were based on orthostatic symptoms, blood pressure, and lower extremity edema.
Ketosis, similarly to fasting or caloric restriction, turns on a group of genes called Sirtuins. When scientists activate Sirtuins in animals, they found that these animals live longer. In addition, Sirtuins can help keep you lean and energetic during the day and sleeping well at night. More research is needed to know if this effect is the same in humans, but evidence seems strong that spending some time in ketosis is beneficial.
As far as the Ketogenic Diet is concerned, I think I am going to have to make a full about-face. As you said, for some people like yourself, you were unable to eat in moderation and feel it is due to your genetic makeup. But wait, there’s one loophole. Have polycystic ovary syndrome (PCOS)? Then the keto diet may help regulate your periods. “Women with PCOS have high insulin levels, which cause sex hormone imbalances,” notes Yawitz. In a small study published in the journal Nutrition & Metabolism, subjects with PCOS following a ketogenic diet for six months noted improvements in their menstrual cycles — and a small number of women became pregnant, overcoming previous infertility obstacles. “This study was very small, so we can’t make recommendations for all women with PCOS based on its findings,” says Yawitz. “And really, any diet that leads to weight loss should help in PCOS.”

Earlier this year Seattle family doctor Dr. Ted Naiman, who has been helping patients with low-carb or ketogenic diets for 20 years, described how his own obsessive compulsive disorder was resolved almost instantly — never returning — on a low-carb high-fat diet. Naiman over the years has seen dramatic mental health improvements in his patients who adopt low carb ketogenic diet. “Definitely bipolar, depression, anxiety, OCD, are all much, much better on a low-carb diet,” he says.
Nonetheless, Fung told Live Science that she thinks the study clearly demonstrates the potential for a ketogenic diet to have a detrimental effect in humans. And, until researchers better understand the risks of those detrimental effects, she suggested that people consider other ways of accomplishing their health goals, such as trying a less-restrictive diet.
In 2006, Czech researchers placed men on either a diet containing red meat or one without red meat, then had them sweat. Women smelled and rated the sweat. Across the board, the red meat dieters produced less attractive body odor. When they had everyone switch to the other diet, the results persisted: Those who had switched to a no-red meat diet produced better-smelling sweat.
The keto diet also appears to help induce autophagy, which helps clear damaged cells from the body, including senescent cells that serve no functional purpose but still linger inside tissues and organs. At the first visit, participants were instructed how to follow the LCKD as individuals or in small groups, with an initial goal of ≤20 g carbohydrate per day. Participants were taught the specific types and amounts of foods they could eat, as well as foods to avoid. In one study they were testing the endurance of the Mice by giving on group MCTs in their daily food, and the other group of mice LCTs. The study lasted for 6 weeks. The total swimming time until exhaustion was measured, and at first there was no difference. As the weeks went by, those fed MCTs quickly began to out-perform the others and continued to improve throughout the testing period.

In March 2018, our friend Vickie, who is a type 1 diabetic, told us about the keto way of life. She shared some interesting data Dr. Ken Berry puts out on YouTube. The things Dr. Berry said made total sense to us and we decided we needed to give keto a try. Both I and my girlfriend decided to give this Keto way of life a try. We officially started on March 5, 2018.

The researchers hypothesized that the positive effects that the ketogenic diet has on migraines are due to how ketone bodies inhibit neural inflammation and enhance brain mitochondrial metabolism. The ketone bodies do this by blocking high concentrations of glutamate (commonly found in both migraine and epilepsy sufferers) and reducing oxidative stress.
In 2009, Drs. Eric Westman and Bryan Kraft published a case study and a review of the literature for a 70-year-old woman with life-long schizophrenia. Diagnosed at age 17, she had experienced daily visual and auditory hallucinations (skeletons that urged her to hurt herself) since the age of 7. She also had obesity, sleep apnea, hypertension, and gastro-esophageal reflux disease (GERD) among a number of other health issues. After only 8 days on the ketogenic diet, the voices and hallucinations stopped. At the time of publication she had continued the ketogenic diet for more than a year, with no return of hallucinations, and had lost more than 10 kilograms (22 lbs).

It is now evident that high carbohydrate diets increase fasting plasma triglyceride concentrations (47–51) and decrease HDL cholesterol concentrations (52–55). These changes are associated with enhanced atherogenesis (55). However, it has been shown that short-term ketogenic diets improve the lipid disorders that are characteristic of atherogenic dyslipidemia (56). It has also been found that sugary drinks decreased blood levels of vitamin E, thus reducing the amount of antioxidants in the body. It has been proven, beyond a doubt, that disrupting the oxidant-antioxidant status of the cell will lead to various diseases of the body (57).
It is important to understand that the statement that carbohydrates are “nonessential” is not only factually inaccurate, it results in adopting a low-carbohydrate diet or ketogenic diet that increases your risk for a wide variety of chronic health conditions that may ultimately shorten lifespan, decrease your quality of life, and accelerate your risk for chronic disease.

Circulating ketone bodies make the blood too acidic, which will draw calcium from the bones as a buffer response. While there are relatively few studies on long-term (more than 6 months) effects of a non-therapeutic keto diet, studies of children on the diet show high calcium levels in the blood, increased bone demineralization and increased risk of kidney stones.

An article published in the New York Times in 2018 explores the use of a keto diet and diabetes type 1. The article points out how many diabetes experts will not recommend low-carb diets for type 1 diabetics, especially if they are children, due to concerns over hypoglycemia as a result of carb restriction and the possibility of this having a negative effect on a child’s growth.
Although this may repel people away, it also provides you with a clear sign that you are on your way to reaping all the benefits of keto and ketosis. As your body continues to produce ketones, it will become more efficient at creating and using the energy-shuttling ketone bodies, beta-hydroxybutyrate and acetoacetate, which will help reduce acetone levels and normalize your breath. In the meantime, while your body is adapting to ketosis, make sure you have breath-freshening gum or spray that you can use throughout the day to cover up keto breath (if you have it).

Unless you’ve already been eating a paleo or primal diet and are somewhat keto-adapted (burning ketones for fuel), it is a good idea “reset” the body in order to regain the metabolic flexibility to go into ketosis or even stay in ketosis despite eating some carbohydrates. Intermittent fasting is one efficient way to do this. During a water fast, ketosis can occur in days instead of weeks or months and often sustains for a while after the fast.

Lower carbohydrate diets (varying amount of carbohydrates in each study) have shown promise in improving A1c and weight management in Type 2 diabetes, is shown to be better than low-fat diets in improving blood pressure and lipid levels, and more. Along with that, more ketosis studies on different disease states are out there and growing. Ketosis has also been explored for its promise, at least in the short term, for metabolic disease, Alzheimer’s disease, and diabetes.


Although this may repel people away, it also provides you with a clear sign that you are on your way to reaping all the benefits of keto and ketosis. As your body continues to produce ketones, it will become more efficient at creating and using the energy-shuttling ketone bodies, beta-hydroxybutyrate and acetoacetate, which will help reduce acetone levels and normalize your breath. In the meantime, while your body is adapting to ketosis, make sure you have breath-freshening gum or spray that you can use throughout the day to cover up keto breath (if you have it).

After the depletion phase, your body will enter a survival mode where it starts to signal the liver to produce ketones from available dietary fat sources and body fat stores. These ketones will replace glycogen as the body’s fuel source in the absence of glycogen. The body will go back to burning glycogen for energy when you start eating carbs again.

There’s many suggested interventions for these short-term keto side effects on the internet, but know that there are potential pitfalls of these dietary supplements. In addition, the study data did not show an adverse effect of a very low-carbohydrate diet on children’s growth, although more research may still be a good idea, according to researchers.
Carbs are still the primary fuel for the body during high-intensity exercise. A ketogenic diet may be better suited to athletes performing endurance sports (like marathon running or cycling). Athletes engaged in more intense cardio (like sprinting, hockey, football) may perform better with a higher percentage of carbohydrates. These athletes may experience a decrease in high-intensity output while following a ketogenic diet, but ultimately, a lowered carbohydrate intake as isn’t “dangerous” for athletes.
They have high likelihood of calcium oxalate and uric acid kidney stones–Kidney stones are uncommon in childhood, yet these kids commonly have kidney stones. The risk in an adult on a prolonged ketogenic effort is therefore high, also. Kidney stones are not benign–they are painful and can occasionally result in kidney damage (increased creatinine, urinary tract infections, etc.)
Is the keto diet safe for diabetics? Most research shows that yes, it is. Work towards cycling fasting days so you’re fasting on 2–3 nonconsecutive days per week (e.g. Tuesday, Thursday and Saturday). Stick to only light exercise or yoga on fasting days to reduce feeling exhausted or hungry, keeping higher intensity activities for your non-fasting days. This approach allows for more dietary and lifestyle “moderation” because the goal isn’t to eat 100 percent “perfectly” all the time.

Holy cow Batman i started keto diet on the 9th of January within 4 days I was no longer on bydureon (weekly insulin) levemir 60units 2x daily and no more Novolog 25 units3x daily all that and metformin 10002x and this all didn’t have my sugars under control amazingly keto diet got sugars in check within four days I’ve been on diet now for 15 days and have honestly never fealt better I’m still green on diet and have had some issues while my body is healing itself (keto flu,low electrolytes very sluggish) .. My aunties actually have been on diet for a year now and were the ones that referred me to it after seeing there transformation i was on Board I’m having my first cheat day cause it’s my bday and other family members haven’t and don’t need the diet so I eat what they make a couple times a year this diet had been literally a life saver to me I’m down to my metformin 2x a day and sugars are at 90-140 depending on when I test after/before a meal with all the meds sugars haven’t been under 200 for the last two years so woot woot to Keto I’ll keep y’all posted thanks for the article my doctor is concerned about ketoacidosis so she’s not on Board and I need forums like this to help me move forward
