All 83 subjects received the ketogenic diet consisting of 20 g to 30 g of carbohydrate in the form of green vegetables and salad, and 80 g to 100 g of protein in the form of meat, fish, fowl, eggs, shellfish and cheese. Polyunsaturated and monounsaturated fats were also included in the diet. Twelve weeks later, an additional 20 g of carbohydrate were added to the meal of the patients to total 40 g to 50 g of carbohydrate. Micronutrients (vitamins and minerals) were given to each subject in the form of one capsule per day (Table 2).
When we eat, we consume either protein, carbs, or fat. Carbs increase blood sugar levels. Protein and fat do not. So eating a low-carb diet IS healthy for diabetics because you eliminate a lot of what causes glucose to rise. I’m not referring to a no-carb way of eating, but a lower carb diet. I’m type 2 and have been eating keto for three weeks. My blood sugar levels are great. No swings — highs or super lows. I was injecting 60 units of insulin prior to each meal. Now I inject 5-10 units. I’m not losing weight, but I feel better and my sugar levels are under control. Most nutritionists and many doctors still haven’t caught up with the science. Low-carb/keto is the best way to eat for a diabetic.
While sometimes it might feel like things are getting worse before they get better, these symptoms should resolve within a few weeks to months of following the tips and program mentioned above. I’m desperate to find some answers~I have no history of sensitivity to anything, no allergies. I understand allergies can happen at any time. I started the Keto diet and in a one month time I had four separate episodes of my tongue swelling~each episode worse than the one before. I LOVED this diet~felt great, handled it with ease. The last episode almost put me in the hospital~my tongue was so swollen it was coming out of my mouth making it difficult to swollen and breath. I had no choice but to narrow it down to Ketosis. All the foods were the same that I have eaten all my life. No new meds. The last episode I took 2 Benadryl and pulled myself out of Ketosis with consuming a Dr.Pepper, and 2 pieces of white toast. I was terrified. Everyone on Facebook in all these Keto sites tell me it has nothing to do with Ketosis….but I’ve tried literally everything I consumed when a was on that WOE and I’ve had no reaction in days. My body feels horrible, I have no energy, my entire body aches. I want back on the Keto diet because my body was responding really well to it~except for my tongue.
I am curious about what has been referred to as “keto rash”. I decided to shift from a GAPS Diet to a ketogenic one. I wasn’t tracking my carbs specifically and was doing well. Then I developed this rash on various parts of my body. I originally thought it might have been either pecan or MCT (C8) that I started having, but I have eliminated both of those. Then I did some research and realized that others have also experienced this rash after starting a strict Ketogenic Diet. There are a few solutions running about–most of which include introducing carbs at higher levels. Antibiotics were also listed. Anyway, I was wondering if you have run across this type of rash as it relates to starting a low-carb diet? Any thoughts, Dr. Jockers?
The same is true for meat. You’ve probably been told to avoid red meat when in fact you should not. Meat is an integral part of any diet including the ketogenic diet. There is no reason to avoid it (OK you’re excused if you’ve been bitten by a lone star tick!). The ketogenic diet reduces cardiovascular risk factors and improves the lipid profile by increasing HDL and lowering triglycerides [9,10].
To be clear, most of the hype you’ll hear about how the ketogenic diet can ward off cancer is taking things too far. While there is a fair amount of research regarding cancer, studies have been limited to animals. Abbey, I appreciate all the work you put into this but there are a few things you missed. Not all oils are keto friendly. Vegetable oils are a huge no-no which was not mentioned. Regular mayo usually contains soybean oil which should be avoided. You also missed that foods with preservatives are a no-no which excludes some cheeses (you said enjoy all the cheese you like). Processed sliced cheese or pre-shredded cheese in a bag at the grocery store contains preservatives. Ever notice how shredded cheese in a bag never sticks to itself but when you shred your own cheese it sticks? Preservatives. Most nuts are okay but peanuts are a legume and should be consumed in very small amounts or avoided altogether. Don’t go crazy either, nuts do have carbs. I don’t eat more than a half cup a day of salted almonds. That said, you could have mentioned that getting salted nuts is ideal as you do excrete more salt and other minerals in your urine (eating keto is diuretic so attention must be paid to salt, potassium, magnesium, etc. as you did mention) so finding simple ways to add salt and other minerals is helpful. I also take a multi-vitamin daily. You mentioned you cannot do high intensity workouts. You are not supposed to do high intensity workouts as elevating your heart rate too high actually stops the fat burning process in your body. Your heart rate should be 180 beats minus your age +/- 5 beats depending on fitness level. A 40 year old obese person shouldn’t go over 135 beats/min during a workout as that is the optimal fat burning window. Higher than that and your body reverts to storing fat reserves thinking it will run out if it keeps this pace. Which bring me to metabolism. Its been long believed a high metabolism is good. A high metabolism leads to more hunger (because you’re burning glucose faster), ingesting more food, and typically gaining more weight. Slowing your metabolism down AND teaching your body to consume slow burning ketones instead of quick burning glucose puts less stress on your liver mainly and on your body in general. There’s a lot more but this is getting long so I’ll finish with this. You said this diet focuses on quantity of food and not quality. This is confusing to me….wouldn’t organic and grass-fed meats, be of better quality than non-organic and grain fed meat? (they feed animals grains to fatten them up….shouldn’t that be a huge warning sign for us?). Is food without preservatives not better quality than food with preservatives? Aren’t beverages with no sugar or artificial sweeteners of better quality than sugar/artificial sweetener-filled beverages? I’ve never seen any keto advocate advise ignoring quality foods, in fact its quite the opposite.
Since carbohydrates do increase insulin and insulin is a storage hormone many people have made large leaps of logic to point the finger at carbohydrates as the primary cause of fat gain and obesity as opposed to overall caloric intake. They also point out that in a keto diet, carbohydrates are not being used as fuel hardly at all whereas fats become the primary fuel.
I have been diagnosed twice with post prandial reactive hypoglecemia since 37 years old ( now 70). Obviously my insulin levels shoot high and then blood sugar drops to 2.2. Can you give me any special advice on the Keto diet with regard to this. I have been diagnosed just recently with lipodema. I have been following diet for 3 weeks and my brain is already alot clearer.

The good news is, if you’re a generally healthy adult, you can probably tolerate the keto diet. “It’s likely appropriate and safe for almost everyone,” says Robert Santos-Prowse, a Clinical Dietitian and the author of The Ketogenic Mediterranean Diet and The Cyclical Ketogenic Diet. “Ketosis is a natural metabolic state that everyone has spent some time in at some point in their lives. It has been proven to be most useful for weight loss and diabetes management, but it could provide benefit for some athletes and also might prove to be useful for many disease states including some cancers and forms of neurodegeneration.”

The good news is, it is not necessary to stay on a ketogenic diet at all times to reap this benefit. Our ancestors went through fast and feast cycles and the body is designed for flexibility. We may be able to reduce cancer risk, prolong life, improve brain function, and benefit from ketosis otherwise by being in ketosis or fasting a few days a week.
MCT oil can be used alongside Coconut Oil  in cooking, frying, sautéing, and baking. If you don’t like the coconut flavor then it is recommended to opt for the “refined” version of Coconut Oil. You can add both Coconut Oil or MCT Oil to any food you like, especially if you like the sweet flavor of Coconut Oil.  A few drops of MCT Oil in almost anything can enhance its richness, and add fullness. MCT Oil is a great alternative to olive oil as a salad dressing, and a good addition to any smoothie.
Depending on how you choose your fats, the keto diet can contain an abundance of saturated fat, which raises levels of dangerous LDL cholesterol and causes atherosclerosis, the buildup of fats and cholesterol in the arteries. Keto cycling is a way to cycle in and out of ketosis while enjoying a more balanced diet on your "days off." One keto cycling approach includes five days of traditional keto diet and two non-keto days per week. Some people choose to save their off days for special occasions holidays, birthdays, and vacations. For best results, eat wholesome carbohydrate-rich foods on your off days, including fruits, starchy veggies, dairy products, and whole grains (rather than added sugars or highly-processed fare).

Glucagon is on the other side of the spectrum; it is insulin's antagonistic hormone. Glucagon is also secreted by the pancreas when glucose levels fall too low. This usually happens when a person skips meals, or does not consume adequate amounts of carbohydrates for an extended period of time. When this happens, glucagon is secreted by the pancreas to break down stored glycogen in the liver into a more usable form, glucose.


In the study, the researchers fed mice a ketogenic diet for several days and expected to find a favorable outcome — perhaps weight loss or another indication of improved health. Instead, they found that the liver began resisting insulin almost immediately and the mice were unable to regulate their blood sugar levels after only three days on the diet. Nine healthy young males participated in this study, which appears in the journal Nutrients. The researchers asked them to follow a 7-day high fat, low-carbohydrate diet that was similar to the keto diet, consisting of 70 percent fat, 10 percent carbohydrates, and 20 percent protein. They also had to consume a 75-gram glucose drink before and after the diet.

I did a lot of research before starting, and am still doing lots of research because I like to be armed with the best information I can have when friends question my choices. Just yesterday I had a friend tell me that our brains need carbs to function (more than what we get from veggies). I hadn’t heard that before so I didn’t know what to answer. Something that makes the keto diet different from other low-carb diets is that it does not “protein-load.” Protein is not as big a part of the keto diet as fat is. Reason being: In small amounts, the body can change protein to glucose, which means if you eat too much of it, especially while in the beginning stages, it will slow down your body’s transition into ketosis.
Also, to the author I do appreciate you stating you are bias up front, but I do get to indulge with some fruit/berries when I want. You dont give up everything forever but you learn to fit them into your macros. I have learned to make Keto ice cream and fat bombs if the urge comes along. I have learned to take keto friendly foods along to potlucks that everyone loves. Keto can be a way to follow forever but everyone has their own needs for their bodies. I am new to this but finding it easier and easier to remain keto

Not going to get into real specifics but like many “trendy diets” some of your claims, despite your applauding effort to site seemingly legitimate sources, are simply inaccurate. First of all MDs are not Nutritionist, they specialize in medicine. Holistic Health coaches specialize in Herbs and using the body to heal itself. Nutritionist/Dietitians focus on proven legitimate evidence to support claims of a basic diet consisting of all 5 food groups for living a healthy lifestyle. Can you live a healthy lifestyle outside of eating the 5 food groups, sure; but you better be well versed on preparation for attempting it. Truth is, any diet, and I mean any reputable diet, on the market can and will work if followed correctly. However, since the beginning of time, civilization has ate wheat as its primary food source and in fact it is still the primary food source worldwide. To eliminate it or complete-whole grains entirely is usually not wise for long term sustainable benefits. Coconut oil is terrible, period. Please never give that advice again. Saturated fat in any form causes arteriosclerosis, cite any heart health clinic or organization in the world for that evidence. Also through starvation, which can occur if the body consistently lacks its basal metabolic needs can and will develop ketoacidosis in a perfectly functioning pancreas. Basically meaning ketosis can develop into ketoacidosis. Again, just trying to provide some further information and clean up your claims. I feel you did a pretty good job touching on a lot of questions/concerns most people have with Keto diets. Like I said, I’m not selling my diet or anything for that matter. I simply want people to realize anything you do requires work, effort and discipline. All in all, it seems more people jumping on the Keto bandwagon are looking for quick results and should be well advised, as you mentioned, on the proper way to eat the right combination of food groups to prevent further deterioration. The right fats, greens and complementary proteins for BCAAs and so on. Truth is a long term Keto diet has not been studied enough to really know if the body is suffering damage long term. At the end of the day moderation, variety and a balanced lifestyle of diet and exercise in combination with the holistic approach (Physical, emotional, spiritual and societal well-being) is the key to long term vitality. Its your lifestyle, not diet that affects your health.

The authors stance against the ketogenic diet is a simple, “its just too hard, I cant live without fruit.” She projects her lack of willpower to her audience. Ketogenic diets are a great way to reduce insulin levels and get to the root of the problem.

While body weight decreased significantly (-8.5 kg) in these 21 diabetic participants, the mean weight loss was less compared with what we observed in the LCKD participants of an earlier trial (-12.0 kg) [18]. Given that the diabetic participants had a higher baseline mean weight than the LCKD participants of our previous trial (131 kg vs. 97 kg), this translates into an even more dramatic disparity in percent change in body weight (-6.6% vs. -12.9%). This lesser weight loss might result from several factors. First, in the current study, most of the participants were taking insulin and/or oral hypoglycemic agents that are known to induce weight gain[20,21] Second, these same agents, particularly insulin, inhibit ketosis, which is strived for in the earliest phases of the LCKD; while it remains unclear whether ketones actually play a role in weight loss on the LCKD, previous research in non-diabetic patients has shown a positive correlation between level of ketonuria and weight loss success [22]. Lastly, compared with our previous study the participants in the current study had more comorbid illness, lower socioeconomic status, and a shorter duration of follow-up (16 weeks versus 24 weeks), all of which are associated with reduced success on any weight loss program [23].

The problem is: it flies in the face of the way we have been taught. Our society (as an American), and our medical community, have preached for years that FAT is bad, and you should limit it. However, recent FACTS beg to differ. So many people rail against the keto diet because they just feel that it can’t be good…after all, you eat so much fat on it, it can’t be good for you! This suggests that we should cycle in and out of ketosis by periodically increasing carbs to avoid this hypothyroidism. Incidentally, the more I read of the much-maligned Bob Atkins’ writings the more impressed I become. He clearly was extremely well-read in the scientific literature and modified and expanded his protocols as he got more information. It has been known for more than a decade that bipolar disorder shares a number of clinical, biochemical and physiologic features with epilepsy and that a similar neurobiology may underpin both disorders. Both conditions cycle, both are risk factors for each other, and anticonvulsant drugs used in epilepsy have been found to be effective in helping manage bipolar illness. 
In the brain, there are some parts that can only take glucose to burn for fuel, which is usually derived from carbohydrates. However, when the body is on a ketogenic diet, the body will enter ketosis, which is a process that produces proteins called ketones from fatty acids in the liver. The brain can surprisingly also take in these special ketones to function. Past studies have shown that children affected with epilepsy who are on this diet have a 50% lower chance of reducing seizures. 16% of that group have also shown to be seizure free. This diet is for individuals with Parkinson’s disease and Alzheimer’s to see if ketogenic diets can also help.

The downsides: While the research is exciting, there's very little evidence to show that this type of eating is effective — or safe — over the long term for anything other than epilepsy. Plus, very low carbohydrate diets tend to have higher rates of side effects, including constipation, headaches, bad breath and more. Also, meeting the diet's requirements means cutting out many healthy foods, making it difficult to meet your micronutrient needs.
Here’s why. All of these effects take time, but a reasonable explanation as to why the keto diet leads to rapid weight loss is due to the loss of water weight. One of the concerns with the keto diet is the loss of muscle mass and the depletion of glycogen stores. Glycogen, which stores our glucose, also stores water, so when stores are depleted, we flush out excess water. In other words, that rapid weight loss isn’t fat, it’s just water.

This diet also entails weight loss because it gets rid of excess water content in the body due to a lower amount of insulin produced. The food is broken down into simple sugars, with excess energy stored as complex sugars. The pancreas produces insulin that helps regulates sugar breakdown in the body. The body needs less sugar in switching to higher intake of fat. The liver can break down fatty acids to energy in ketogenic diets. Due to eating less carbohydrate or sugars, the production lowered. In a study, 90% of all type 2 diabetics were able to lower or completely eliminate their diabetic medication after 6 months of being on a low carbohydrate diet.
Test for allergies and intolerances. For suspected allergies and intolerances, It is best to try an allergy test and an elimination diet that removes all common allergens and intolerances (wheat, eggs, fish, dairy, nuts, peanuts, shellfish, soy, and nightshade vegetables) for a period of time (3-4 weeks). If you are noticing positive changes and want to start adding these foods back into your diet, choose one at a time and tread slowly. Take note of how you feel. If you are still feeling great, move onto the next one. If at any time you experience adverse reactions to a certain food, chances are you have an allergy or intolerance to it.
The targeted keto diet is popular among athletes and active individuals who live a keto lifestyle but need more carbs. It allots an additional 20-30 grams of carbs immediately before and after workouts to allow for higher-intensity exercise and enhanced recovery. (The total carb count comes to 70-80 grams per day.) The best options include fruit, dairy or grain-based foods, or sports nutrition products. Because the additional carbs are readily burned off, they don't get stored as body fat.
The one important caveat: Eating keto also ups the risk for diabetic ketoacidosis, a life-threatening condition where fat gets broken down too fast and causes the blood to become acidic. It’s much more common in people with type 1 diabetes, but if you have type two and are eating keto, talk with your doctor about what you should be doing to diminish your risk.

If you decide to follow the diet, you’ll want to have an individualized meal plan, stresses Dr. Gonzalez-Campos. “The best possible clinical outcome is for each individual to meet her nutritional needs from well-balanced meals,” he says. “Ideally, we should all have the benefit of medical nutrition therapy and we should all avoid extremes in nutrient restrictions.” 


"Most people who wind up trying a ketogenic diet and then deciding not to continue do so because of the emotional and lifestyle consequences," Turoff says. To put it simply, people miss eating carbohydrates. "That doesn't mean that you should be eating pizza or cupcakes every day, but what about having a sweet potato with a meal, or beans in a chili? Or fresh watermelon in the summer?" Turoff asks. "Whether or not we want to admit it, food plays much more than just a physical role in our lives and having such restrictions on the types of foods you can and can't eat can really take a toll. It might be easy in the short-term to go for carb-free foods but at a certain point, the thought of not being able to eat your favorite foods again can become daunting."

Even if you don't have a history with eating disorders, keto can still leave some people (although certainly not everyone!) with negative feelings around food. "For the vast majority of people, keto is not sustainable, meaning they don't stick with it for more than a few months," Brown says. "Not 'succeeding' with this diet and returning to one's usual eating habits can trigger feelings of guilt and failure." Some athletes swear by the ketogenic diet, not just for weight loss but for improved performance in their sport, as well. Carbs are still the primary fuel for the body during high-intensity exercise. A ketogenic diet may be better suited to athletes performing endurance sports (like marathon running or cycling). Athletes engaged in more intense cardio (like sprinting, hockey, football) may perform better with a higher percentage of carbohydrates. Researchers found that overweight adults who lowered carbohydrates and added more fat into their diets burned about 250 calories more each day than people on high-carb, low-fat diets. The study is impressive because it's the largest, most expensive, and controlled study of its kind.
On a low-carb ketogenic diet, your largest dietary food source should be healthy fats. Make sure to choose fats from a variety of sources, including oil, butter, fish, nuts, and seeds. Your body needs the different nutrients each type supplies. No more low-fat or non-fat foods! Remember: Not all fat is bad. Healthy fats are non-hydrogenated, cold-pressed, and plant- or seed-based.
As with any diet, precautions need to be taken. Pregnant women and those with kidney disease are not good candidates for this diet, and some people with diabetes may find that the diet increases their insulin resistance. Dairy can often spike blood sugar, so avoiding the dairy in a keto diet and taking a Vitamin D supplement might be a better option for some people. It’s important to pay attention to the way your body responds and realize that no diet is a one-size-fits-all model.

"For the vast majority of people, keto is not sustainable, meaning they don't stick with it for more than a few months," Brown says. Adding salt to food might be new to you, since most people are used to being told to limit salt intake. However, when you’re eating a ketogenic diet of less than 60 carbohydrates each day, you’ll need to make up for this loss of salt. That being said, those with high blood pressure who take medication should check with their doctors before making a change.
If you want to use a ketogenic diet as a health tool, it would be wise to do so for no more than a few weeks at a time, as nobody knows how long is too long. And, of course, the period of time during which ketosis is safe can vary from individual to individual. Increases in cholesterol levels need discussion too. We do see temporary increases in cholesterol levels often as individuals transition onto a ketogenic diet. There are various ways to practice intermittent fasting on keto, including some that are less likely to trigger side effects like fatigue or cravings. Crescendo fasting gives you a break from fasting throughout your week, but is still very helpful for achieving the benefits of IMF. Dr. Cabeca and other experts, such as Amy Shah, M.D., advise their patients to check their urinary ketone levels (using ketone strips) and to aim for testing positively for ketones about three days per week.
The good news is, if you’re a generally healthy adult, you can probably tolerate the keto diet. “It’s likely appropriate and safe for almost everyone,” says Robert Santos-Prowse, a Clinical Dietitian and the author of The Ketogenic Mediterranean Diet and The Cyclical Ketogenic Diet. “Ketosis is a natural metabolic state that everyone has spent some time in at some point in their lives. Circulating ketone bodies make the blood too acidic, which will draw calcium from the bones as a buffer response. The one important caveat: Eating keto also ups the risk for diabetic ketoacidosis, a life-threatening condition where fat gets broken down too fast and causes the blood to become acidic. It’s much more common in people with type 1 diabetes, but if you have type two and are eating keto, talk with your doctor about what you should be doing to diminish your risk.

Plus, "some research has found that athletes on a keto diet become less efficient in their exercise," Brown says. Because of this, the researchers involved in the study recommended that instead of going for a keto diet, athletes who want to take advantage of their body's ability to use fat as fuel alternate between training in a carb-depleted state (for example, doing a fasted workout in the morning after not eating carbs the night before) and eating carbs as normal. I have been working more seriously on my health after a major health crash 5 years ago, possible Rocky Mountain Spotted Fever/Lyme. Organ functions are getting better including heart, liver and gallbladder. Pancreas is at stage 3 insufficiency. I’m concerned about a high fat, protein diet. Still have IBS and gut infections. Thinking that just bone broth and easy to digest foods might be good at this point for healing?
However, we need to consider that most of these studies failed to equate for both calories and protein, and it is likely that if similar weight loss was achieved, these health outcomes would look similar regardless of the diet used. The struggle as a prescriber is that you have to follow the “standard of care,” lest you open yourself up to a lawsuit. Mastering Diabetes: Studies conducted in tens of thousands of people over 5+ years indicate that low-carbohydrate diets increase your risk for cardiovascular disease, hemorrhagic stroke, hypertension, atherosclerosis, diabetes mortality, obesity, cancer, and all-cause mortality (premature death). No matter how you slice it, low-carbohydrate diets trick patients and doctors into believing that ketosis is an excellent long-term dietary strategy, when in reality the consequences can be disastrous.
You have about 160,000 Kcals of fat available to burn – producing your own glucose in the process when needed (for anaerobic bursts). In contrast you only store around 400 to 500 Kcals of glucose/glycogen ready for use so if you are not adapted to burning fat then you are physically dependent and addicted to eating carbs. What about heart health and the keto diet? Previous older schools of nutrition would purport that a diet rich in fats (specifically saturated fats) would be detrimental for heart health, but more recent research suggests that saturated fat is not as bad as previously believed.  There is actually a tiny little bit of evidence that a keto diet may improve triglyceride, HDL and LDL levels. Like here and here. An even more recent study found that a keto diet improved triglyceride, HDL and LDL levels. We’ll definitely have to wait to see how that research unfolds because there is definitely a lot of competing elements at play.
I am beside myself. I’m at the point that I’m afraid to eat anything. It goes right through me!! I’ve had liquid diarrhea for 3 weeks now and don’t know what to do. I’m up through the night and it’s really messing with my sleep. I have breast cancer and I really need to do this diet. I’m very worried. I have been taking psyllium husks but I’m afraid to go overboard. Is it ok to take that every day? Thanks you!
At this point, I’m searching for the secret sauce that will allow me to follow Wheatbelly or Keto or any other low carb diet, but not develop any health problems. When you have 80 lbs to lose, it’s discouraging to be told that you can’t be on the only diet that has ever worked for more than a few months. I’ll need a year or more just to get to goal weight.
Fresh fruits supply only a small fraction of the fructose Americans consume (the biggest source is sugary beverages). Sugar in fruit is accompanied by healthful nutrients and antioxidants, as well as fiber, which slows absorption of fructose so it’s highly unlikely that the fructose in even very sweet fruit could have any undesirable health effects.
